First Visit

We offer 2 types of appointments:

Consultation

  • Consists of an examination followed by an explanation of your diagnosis and treatment options
  • Treatment, if needed, is scheduled for another day.
  • The consultation appointment has its own associated fee

Treatment

  • Consists of a consultation followed by treatment, if required
  • In certain instances, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your initial visit:

  • Your referral slip and any x-rays if applicable
  • A list of medications you are presently taking
  • If you have dental insurance, bring your insurance information. This will allow us to help you process any claims.
  • To help save time prior to your treatment, our patient registration form may be submitted online or printed from our website and completed prior to your arrival.

IMPORTANT:

All patients under 18 years of age must be accompanied by a parent or guardian at the consultation visit.
